Slovak citizens can submit a written request for a change of first name or surname through the Consulate-General. Some changes, such as taking a spouse's name or reverting after divorce, need no permission.

Where to apply in Australia

Contact details for every office are on the page of the embassy and consulates of Slovakia.

How to apply

Submit the written request with required documents | pay cash AUD at submission | for return by post, include a prepaid registered-post envelope, otherwise collect in person. The decision is taken by the district office in Slovakia linked to your last permanent residence (Bratislava if none).
