A parent or guardian can apply online through the Polish government e-service for a passport for a child under 12 who already has a PESEL number, then collect it at the consulate.
Where to apply in Australia
Consulate General of the Republic of Poland in Sydney (10 Trelawney Street, Woollahra NSW) covers Australia except the ACT; the Embassy in Canberra covers the ACT. Honorary consuls do not issue passports.
Contact details for every office are on the page of the embassy and consulates of Poland.
