Polish passport for a child under 18 (valid 5 years under age 12, 10 years from 12). Both parents or guardians normally take part; one parent may apply alone with the other's notarised consent or a court ruling.
Where to apply in Australia
Consulate General of the Republic of Poland in Sydney (10 Trelawney Street, Woollahra NSW) covers Australia except the ACT; the Embassy in Canberra covers the ACT. Honorary consuls do not issue passports.
Sydney accepts cash, card or contactless; Canberra cash only.
Contact details for every office are on the page of the embassy and consulates of Poland.
How to apply
Book an appointment on the e-Konsulat system (e-konsulat.gov.pl) | parents/guardian attend; children under 5 need not attend | the employee completes the e-form and parents sign on a pad | pay at the post | a parent or guardian collects in person, postal delivery only in justified cases on written request
Applications are handled through e-Konsulat (Polish MFA online booking system), https://www.e-konsulat.gov.pl/.
